Dear Former Allcare Patients,
We are greatly saddened by the recent events you have experienced as a result of Allcare Dental’s abrupt closing. Patients who have invested their time, hard-earned money and, perhaps most importantly, trust in a dentist should never have to endure such a situation.
To help serve your dental needs going forward, we at Afinia Dental have voluntarily taken ownership of the dental records and lab cases from Allcare. We are not affiliated with Allcare and have not received any monetary compensation for taking over the care of your confidential records. Below we have outlined some important details that will hopefully ease some of your concerns:
Afinia Dental will meet individually with each former Allcare Dental patient to discuss treatment options for continuing care. There is no fee for this appointment or examination and x-rays should you choose this option.
- Be assured that your records are safe, secure and under the care of a licensed dentist.
- We will do our best to work with you, to gain your trust and to hopefully become your dental provider. In the event you wish to seek dental care elsewhere, we will arrange for you to get the copy of your chart as quickly as possible. We ask for your patience with this process, as we have taken on a large responsibility in a short period of time.
- Please be aware that we cannot be responsible for the accuracy or completeness of your dental records. We are simply copying what we received via Allcare. Afinia Dental has become the custodian of these records; legally, we have to maintain the original copy.
- You can obtain a copy of your chart by filling out the form attached to this page and returning it to us via fax, mail or email. For your privacy and safety, we operate under HIPPA and Federal Privacy Regulations to ensure your chart is not given to any unauthorized party. We will work to have your chart ready for pick-up at our office as quickly as possible, within at least 10 business days. Because of constraints on the size of our current office, we will be storing Allcare patient records in a secure off-site location and this is the reason for a slight delay in getting you your needed copies.
- We have little to no financial data about your individual account.
- We do have your dental information, x-rays and lab work (if Allcare had it in your local office).
- We will not give refunds for any work Allcare has or has not completed.
- We will not warranty or offer complementary care on any existing Allcare dental work.
- We will be happy to provide you with safe, stable preeminent dental care in the future.
- We will, however, meet with each patient individually to discuss treatment options for continuing care. There is no fee for this appointment or for examination and x-rays should you choose this option.
